Recreation Forms

Program Registration Form: Please fill out this form and submit to the Recreation Department before starting any program or activity. Program Registration Forms are required for every program even free programs offered by the Recreation Department.

Heart of Virginia Festival 10K Run & 5K Walk Registration Form: Please complete and return the registration form along with payment to the Town of Farmville Recreation Department by the deadline listed.

Summer Day Camp Registration Forms: All summer camp participants must submit a Summer Camp Registration packet before the start of their summer camp in order to participate.

Youth Football & Cheerleeding Registration:The Youth Football and Cheerleeding registration form must be submitted to the Recreation Department with the required documents in order for to participate.

Youth Basketball Registration Form: Interested Youth need to complete a Registration Form. The Recreation Department is sponsoring the PEFYA Youth Basketball League for the 2011-2012 season.

Adult Basketball Waiver Form: All participants must complete and submit this  form before participating in the adult basketball league.

Youth Sports Registration Form: A Youth Sports Registration Form must be completed and submitted to the Recreation Department before participating in any sport offered by the Recreation Department.

2012 Jingle Bell Run Registration Form: All participants must complete and submit this registration form by the deadline of November 30th.
