Program Registration Form:

Please fill out this form and submit to the Recreation Department before starting any program or activity. Program Registration Forms are required for every program even free programs offered by the Recreation Department.

Summer Camp Registration Packet:

All summer camp participants must submit a Summer Camp Registration packet before the start of their summer camp in order to participate.

Youth Sports Registration Form:

A Youth Sport Registration Form must be submitted to the Recreation Department before a participant can participate in any sport offered by the Recreation Department.

Adult Sports Registration Form:

A Adult Sport Registration Form must be submitted to the Recreation Department before a participant can participate in any sport offered by the Recreation Department.

Pre-Fitness Evaluation Form:

A Pre-fitness Evaluation form must be submitted to the Recreation Department and/or Instructor before a participant can participate in any fitness program offered by the Recreation Department.

Program Evaluation Form:

The Recreation Department asks that all participants to please fill out a Program Evaluation form at the conclusion of the program. This provides the Recreation Department with valuable feedback and information to help improve and maintain certain programs.

Program Proposal Form:

If you have a program you would like to offer to the public or are interested in the Recreation Department offering a certain class or program please fill out and submit a Program Proposal form to the Recreation Department.
